Hand over Fist...

What the hell does this stupid phrase which I tend to use with great regularity mean?

I mean, I get the concept of it. In the context I'm talking, if for example someone says they're making lots and lots of money I've always taken it to mean they're making money "hand over fist".

My question is, where did this phrase come from, and what did it originally mean if it is a derivative of another phrase.

Not an "official" answer but:

Probably from a naval/seagoing concept of climbing (or pulling) a rope quickly.

You use your open hand to grab the rope above the section you are holding with your closed hand (fist).

In terms of making the money hand over fist, think of using the same motion to pull the piles of money closer to you. Holding money with one hand, while the free one reaches out to rake (grab) more of it and pull it in.

That's what I always thought it may have derived from. Possibly and early bullying phrase and somehow wound up as a term of financial success.
