The Tori Amos cover of Smells Like Teen Spirit was a B-Side to a song off Little Earthquakes (I forget which one)

The Pearl Jam cd Versus was originally going to be titled "5 Against 1" but was changed at the last minute. So on the day it was released some pressings said 5 Against 1 and some didn't have an album name on the spine. After that, all of them say Versus.

Correct, it was on the EP "Crucify" which also included "Winter" from Little Earthquakes, "Angie" (Stones cover), and "Thank You" (Zeppelin cover).
