The Costello catalog has been purchased the most, thanks to his constant label moves.
Otherwise, it's between Dylan's Blonde on Blonde and the Replacements Tim. Tim has been purchased a handful of times because I've lost it or worn it out. Blonde on Blonde started with a vinyl version, followed by the original CD that had an edited version of Sad Eyed Lady on it. When that was expanded to include the full version, I had to have it again. Then a SACD version was released, followed by a remastered hybrid surround sound SACD.
Oh, and also Royal Albert Hall fits in there somewhere. First, the white label vinyl boot, then another version with a "real" cover followed by a bootleg box set that had it. Then at least two bootleg CD versions, followed by the official release.

Never/none
There are a few albums I owned on vinyl, cassette, and then CD, but the one I bought more than one copy of just to give it to people because it was that important to me was Willie Nelson's "Spirit."
I've never met a single person who hasn't absolutely fallen in love with it once they've listened.

I don't think I really got in to music until CD's so I never had to upgrade anything. The only album I can think of would have to be:

That's only because I lost the disc but still had the case. Years later I found the disc so now I'm stuck with two copies.

I've never heard "Spirit". I guess that answers the question what you can get me for Christmas.

There are tons of albums I've bought more than once, but I think the big winner is Frank Zappa's We're Only in It for the Money. I think I've owned around 5 different copies of it on CD and vinyl.

Bought it on LP. Bought the Canadian press for the different track. Bought the cassette for the car. Wore the cassette out; bought another. Wore out both albums. Bought 2 more. Bought it on CD when it was part of a 4 CD box set. Girlfriend took that. I stole another from a store. Lost that. Wore out 2 more vinyl copies. Bought it as a single CD reissue. Downloaded then sold CD then lost album in corrupted iTunes debacle. Bought the CD again. I also currently own 5 copies on LP so I can replace it like shark teeth.

See above and multiply by 3.
I've bought several repeats mostly to convert from vinyl and/or tape to CD.
This one I bought a couple of times because I got the stereo mix and the sessions box set.

The only CD's I've had to replace due to damage were Beck's Odelay, Soundgarden's Badmotorfinger & Black Hole Sun and the first 2 Oasis albums.
I upgraded a couple dozen cassettes to CD's over the years, but most of those tapes were bought dirt cheap so I don't really count that.
I've definitely upgraded a bunch of albums due to them being re-released with remastered editions. Most of Elvis Costello's albums, the Replacements' albums, 3 Pulp albums, both My Bloody Valentine albums, Matthew Sweet's Girlfriend, Whiskeytown's Strangers Almanac, the Mission of Burma reissues, the continuing Pavement reissues, the Sonic Youth reissues, the Jesus & Mary Chain reissues, the New Order reissues (I replaced the Joy Division albums with the comprehensive remastered boxset quite a few years back), NIN's Downward Spiral, DJ Shadow's ...Entroducing...I'm sure there's plenty of others I'm forgetting, but I'll get them almost every time if there's remastering involved., especially bands from the 70's and 80's.
I don't know if it counts, but I tend to pick up best-of's by bands I have most or all the albums for if I think the albums could be remastered, since the best-of's tend to sound better.

Pretty much all of the CDs I own are stuff I previously had on vinyl. I lost all my records (That's what we used to call them back in the 70s) in various moves over the years.
