I've got a drjoek question all lined up...
for example:
Dear Dr.JoeK
I was recently at the dentist and they tell me I'm grinding ... but I don't think I do it at night... and I think it has to do with the fact my jaw is totally misaligned from previous rambunxious youthful activity.
Now it appears I'm nearing wearing away the entire left side of my teeth's enamel.
I've been told that realignment is possible w/ a chiropractor or with special dentistry... have you heard of the chiro thing? B/c it sounds a lot easier.
Thanks,

See this is the type of thing that a forum could be for.
As far as grinding your teeth its something you may not be aware of at night. If you wake up with a sore jaw or aching teeth you probably are. If not you may still be. As to realignment that is a very complex process that would be very expensive and in my opinion not really successful. I don't put much stock in chiropractics so I can't comment on that. The easiest approach would be to have a mouthguard made to prevent any further damage to your teeth they can be made pretty inexpensively and for the long term will save your teeth. I use a mouth guard at night and also when I drive because for some reason thats when I clench
Good Luck

Tooth # 15 is a pretty far back tooth so easthetics really isnt an issue so it would come down to personal preference. I don't buy into being concerned about the whole mercury concern there is plenty of hard data that shows mercury release from filling is not happening. White filling now a days are very good and stable. Given a choice I think I would do the white for a couple os small cavities. Good Luck.

on that note, I always have been told that the amalgam fillings hold up better than composite ones, I did use them for molars cause they cost like a third of the white fillings yet they still seem to break.
what type of filling will last the longest?
I will have to find a dentist soon, I haven't been in a while and I know that's not a good thing at all.
No dental insurance and other obligations really stinks.

As a rule amalgam does not wear out the way white fillings do although they have strengthened then over the years.white fillings have the added bonus of bonding structures together for added support rather than just filling a hole Any filling that gets real big no matter the material will tend to break hence the need for a crown. As to going to the dentist as often as is financially possible. It definately helps to go as best you can since dental problems don't wait. But absolutely its a "luxury" item that has to be delayed in these times. But if it can be done it may be less costly in the long run.

Well if helping a mod out can't get me points toward a forum nothing can.
If Sean hasn't worn his retainer in 4 days he won't do irreparable harm. It may be very tight when he is able to put it back in, but any movement of his teeth in this short time will be corrected shortly. He should go back to wearing it as soon as possible though.
How long has he had his braces off? The longer they have been off the less of a problem it should be.

Rich
GERD or Gastric Reflux Disease can certainly cause advanced tooth decay. The acids in stomach fluids are highly abrasive and will eat away dental enamel. Its not a carious or cavity thing its actual "burning' away the enamel. Teeth will have a flat smooth appearance on the tongue side and will be more suseptible to cavities since the enamel is compromised. As an orthodontist I see alot of young girls and have picked up eating disorders in girls who are bulemc. I have had to point it out to a few parents and try to get them some help.
The only dental suggestion I can make is to keep your teeth as clean as possible while dealing with reflux so that cavities aren't worsened. Dr Steve can give you some advice on treatments of GERD

I have dealt with reflux and chronic gastritis since I was 22 in the Army when I began puking up blood clots. I am surveyed every 2-3 years at 35 to make sure no cancer is formed, but it always shows a nice yellow fundus meaning gastritis. Prilosec had been and still is a routine of mine.
I brush my teeth at least twice a day, floss twice a week, but my bottom front teeth are going to fall out sooner than later. This sucks and I am gald I am covered under the military and the VA, but I wonder if they will really do me justice cosmetically.

I have a question; I lost a piece of filling last night. I haven't been to a dentist in over a year due to no coverage.
1) how long can I wait to get that filling replaced? I am not in any pain and rather find a good new dentist willing to take payments and all than rushing into some office that does not deserve my business. (I have had nothing but bad luck with dentists in the US.. all my root canals over the years were screwed up and had to be redone after they became very infected a year later.)
2) do you have any fellow dentists in PA that are recommendable? Is there a good directory to search one out in?
Thank you.

The sooner the better to get a lost filling replaced but you should have a little time. Pain is an indication that it needs immediate attention but dont wait for it to hurt then it will be a bigger deal. Many dentists will set up a payment plan on a filling if its expensive. Where are you in PA? I can find a few names for you.

I also looked back into a dental plan since I had one a few years ago and it saved me 50% on a root canal (which went sour a year after anyway)
I used yo have avia dental and I think their rate is decent as far as covering a cleaning and routine visits and the fillings seem pretty decently discounted as well.

Most dental plans cover the basic cleaning fillings etc pretty we'll. Its when you get to crowns surgery where there's a lot of out of pocket
Sometimes you can never tell who's good on your plan if you have to go in network. But now a days most all dentists participate in some plans. Used to be just the hack guys were in plans to get patients. But now you have to take a few plans to stay busy. So your chances of finding good care are better.

well, if you can recommend a few dentists I would greatly appreciate it. I need to get this done soon and most likely a few more fillings need repair.
I am also getting frustrated by my top back molars cause the tiny gap between them catches everything from oranges, apples, spinach, lettuce or chicken yet is tight enough to break flossers so wonder f anything can be done about that as well.

neither of the ones you recommended are on the dental plans lists. I called one and she said that most dentists in Easton don't participate in the plans :(
So I asked if they would just do a filling and not force me to do an entire exam with x-rays and all and they said that she is obligated to do it and have a full exam done.
I guess I am going to have to save up and pay up.

Wow, I find this utterly messed up. Ideally you want to see the whole picture, and address any and all dental health issues that are present, but we don't live in an ideal world. This is something that needs to be addressed now. It just comes across as not caring about a person's dental health, and the fact that it can get worse if left untreated, and more about selling as many services as they can push on the patient.

It just comes across as not caring about a person's dental health, and the fact that it can get worse if left untreated, and more about selling as many services as they can push on the patient.
I don't know, it said it has something to do with liability... if I had x-rays that are no older than 2 years old from an old office they would accept them but else they need new ones.
I don't know exactly when my last x-rays were, but given that Lukas is 3 now my guess is they are at least 2 1/2 to 3 years old as I had a lot of stuff fixed after I had Lukas.

Did they say they just want an x-ray of the area? Or do they want you to have a full mouth series, and comprehensive exam? When we treat an emergency we do what is called a problem-focused (aka emergency exam), that means we just look at what your complaint is. This is much different than having a comprehensive evaluation, which looks at everything (this information is used to formulate a treatment plan). A comprehensive exam is ideal, but problem-focused exams are performed for a reason, to get the person out of pain/on the right track/etc.

neither of the ones you recommended are on the dental plans lists. I called one and she said that most dentists in Easton don't participate in the plans :(
So I asked if they would just do a filling and not force me to do an entire exam with x-rays and all and they said that she is obligated to do it and have a full exam done.
I guess I am going to have to save up and pay up.
Lets try this in the opposite direction
Do you want to PM me the names of some of the docs within a decent distance of you that ARE on the plan and I can ask around for some feedback?
As far as the exam and xrays I wouldnt be confident treating someone with x rays that are 2 years old but if someone has a problem that they want to treat a single filling thats broken most offices would oblige to prevent further problems. You do want to have a comprehensive exam done when its economically feasible.

I already pm'd you a name of a doc that is accepting the plans.
I am not afraid of dentists or dental procedures, it really is a money issue.
If they would allow for payments rather than make me take a credit I would gladly go in and have everything taken care of the best way they can.
As a matter of fact I would have a few fillings redone because they already look like they will fail soon and most likely get some crowns done to a few of my teeth.

Heres my suggestion
go see the guy on your plan to take care of the broken tooth filling See if you feel comfortable with the office and their policies and the patient care then you can go from there. At least you will have taken care of the immediate problwm so it doesn't progress and you will get the feel of the office and will know if you want to procede with more work
I'll see if Ican find out anything about hm if anyone I know knows him.

Most sedation can be coded as a medical procedure. I would suggest you get nitrous instead though. It can give you all the effect you need to get the work done but then its gone as soon as they turn it off. Nitrous can be administered at a significant enough level to make almost anything managable and you don't have all the risk associated with sedation. Probably alot cheaper too.

I asked my dentist to do the nitrus thing last time, and she said "we dont do that here". Is there like some code word I have to use, or are there places that specialize in that? How do I go about looking for a dentist that will do that?

Some dentists don't want to pay the extra cost of insurance, machinery etc to offer nitrous. Most offer it but i know a few who don't. Ive actually heard a few woman dentist in child bearing age who don't want it in the office.Call an office and ask if they offer Nitrous oxide

Sleep apnea is a sleep disorder characterized by pauses in breathing during sleep that lasts long enough so that one or more breaths are missed, and such episodes occur repeatedly throughout sleep.
If the apnea is caused by an obstructed breating problem versus a neural(brain) impulse than an oral appliance has been shown to help by opening the airway to allow air flow while asleep. Unfortunately there is a certain amount of trial and error to find one that works. I recommend you and your dad go see a dentist who has some experience with this and get fitted for one that will help if it is an obstruction problem. If its a brain based problem an appliance may not help.
As far as the over the counter ones I have not seen many people have much success with them.
Good Luck

I think there are plenty of procedures you can have done during pregnancy, though sometimes it gets worse during and it is better to wait until after when your hormones are balanced out again...
Is it a filling that came out or actual tooth?
They may be able to give you a temporary filling and you can get Novocaine too, they have shots without epinephrine in it which should be safer for you too use.
In any case, if it's the tooth and a large area it is exposed you may want to get it filled and built up temporarily, if it goes too deep they may want to do a root canal and you really want to avoid that.
Pregnancy is such a bitch... some women lose hair, others get skin problems and others have trouble with their teeth... some get all side effects... not to mention the back and ligament pains... I swear whoever said pregnancy is wonderful and the women glow... punch them in their gut!

I have a question for you:
After just having had my wisdom tooth removed I have to say I am in very little pain compared to what I expected it to be and so far it seems healing good.
I did take antibiotics for almost 5 days now so that might help the case.
Here is my question; a friend suggested sucking/ biting on a tea bag to promote healing and stop bleeding. I did not do it cause I was no longer bleeding... but I wonder if this is one of these home remedies like when old people told me to bite and chew on parsley to control pain on an infected root canal many years ago...
I have stuck to warm saltwater rinses and more or less a liquid diet for almost 2 days now and I feel like it is going well.

Based on yesterdays show Dave said you were at the dentist so I assume you were helped out with your tooth. If they took care of it that's great No X rays I assume. Not everyone has problems with wisdom teeth so that's good news. There are things in Tea that do promote healing so they will do no harm and can help. Stay away from smoking and sucking straws to avoid dry socket ie lost clot.
I haven't ignored the PM I will get on that as soon as I get back to the office.
Yo drjoek! Saturday I started getting a pain in one of my teeth when I eat or drink anything hot. Apparantly cold doesn't bother it. I'll be setting up a dentist appointment just in case. Usually Cold sensitivity is the first sign of a cavity. Hot sensitivity can be due to a sudden trauma leading to a little extra blood flow to the tooth which is a confined space and has no room to expand. Likely not anything to bad if you havent noticed anything until now. definitely get it checked could be a cracked tooth also. A quick xray will ease the concern.

Hey Doc,
I had a tooth pulled about 6 months ago. I remember the tooth being pulled, and then feeling like there was a little sliver of tooth left in there. I think the dentist didn't do a good job of extracting it fully. he said not to worry about the piece, and it would do no harm.
Just a few days ago, and since then, I have had a bad pain coming from the area where that tooth used to be. When I press on the skin around that area, or the hard parts of my gum, it feels like I am pressing on a tooth that is infected or abscessed. I was wondering if it could be that leftover sliver of tooth causing it, and what I should do to get it checked out.

Sometimes when a tooth is extracted a small piece of the root may break off and remain in the gums this piece will invariably work its way out as the body has a protective response to do this. It should resolve over a short period of time by pushing its way out.

I have no massaging chairs here since I would use them my self and get no work done!
I cant imagine trying to drill teeth while a massage chair is going. Did you ever see Steve O in Jackass get the Tattoo in the back of the Jeep moving at full speed? Thats what I would fear.
I tend to use novacaine for everyone as a perk for myself You can drill a tooth with out any but the occasional twinge can make a patient real nervous and move their head abruptly. Enamel has no nerve endings so it is pain free but the inside of a tooth has nerve endings. I assume it was pretty small if she wanted to do it without.

Well the pain was from hot and cold, but not when I bit down on it. They did take an xray, and saw the small cavity... then she had the air blower and some dentist ice spray in a can to check where it hurt.. I does feel better and the pain is getting less little by little.. I guess we'll see.

If this is the first time a tooth is drilled vs replacing an old filling sensitity can last longer.

OK it has been 1 week since getting the filling and it's still sensitive to hot and cold. I call them today and the receptionist says, "That's because you need a root canal". So I'm like "What are you talking about? I had a toothache, you say it's a cavity, you filled it and it still hurts" She then tells me that there was a small fracture in the tooth and it probably got worse.
To this I should mention:
My original visit, she said "you have a very small fracture in the tooth, it should be fine. when you come back for your cleaning I'll see if it gets bigger". That was all she said about it.
Not once did she say "You need a root canal". All she said was, "You have a very small cavity, do you want a white or silver filling"
Does it seem like she's either incompitant, or trying to make money off unnecessary work? or am I just paranoid?
Sorry if this is written like a child would, I'm pissed..

Seems like an odd situation. If they filled the tooth how do they now think its fractured? Why fill it if it had a fracture? Have they reexamined the tooth before making this fracture determination? In addition a root canal isn't nessasarily going to take care of a fracture. I think more info is needed before you jump into a root canal.

OK when I went there she did a bunch of tests (cold, pressure, air) examined the tooth and took an xray. First she said you have no cavitiy and no fractures. Then she did a few more "Does this hurt" poking around, when I said yes, she said let me look again. Then she said you have a very tiny cavity I can fill it. You also have a very small fracture that looks like nothing, but when you comeback for your checkup, I'll xray it again to see if it increases in size.
Then I got the filling and was on my way.
I'm thinking of going back to my previous dentist. It's a bit of a drive comapred to this one, but I know they do great work.
Also I should mention, this place made me a little wary when I realized there were no examinations rooms. Just 6 chairs sectioned off by cubical style half walls. Looks like a nail salon or something. That should've been my first sign to leave I guess..

Go see your old dentist for peace of mind. A fractured tooth should be addressed right away sincs as a fracture it can get worse and end up in a lost tooth.

Thanks doc.
I just cancelled the quack dentist. And I'm calling the previous one.

I hear ya, but I don't even want to go back there.. I think it'll feel like going by an ex-girlfriend's house to pickup some DVD's I left behind.
PLUS, her "Xray" setup looked kinda chinsy.. Windows crashed 3 times while trying to get a shot of my chompers.

Hey, drjoek, My dentist (awesome guy) wants to whiten my teeth. I feel like it might be a waste of time because I love coffee and tea. They're not too yellow, but now that he's mentioned it, I'm a little sef-conscious about it. Could I get my teeth whitened and still keep my beloved coffee?

Tooth whitening is by its nature a temporary proceduce which will be hastened by coffee drinking. If after a professional whitening you get a home kit with a night tray you can stay ahead of the coffee staining.

I have been wanting to whiten my teeth too but I am afraid with the one crown I have in the upper front row that the others may be whiter than the crown and then it looks weird...
Going with the whitening thing. Which whitening product to you and 3 other dentist stand by

So while at the dentist the other day I once again noticed the ads for Rotadent and it stating to be better than Sonicare toothbrush.
I have been using a Sonicare for a few years and I am fairly happy... except brush heads are expensive and I still have issues with some tighter spaced teeth.
Do you recommend one over the other? What is the general view on electric toothbrushes?

I actually reccommend the Sonic care only from experience of my patients. We have sold/given away a few of the rotodents in the past and the patient feedback has been better form those who use the sonic care. they both work very well but the sonic care is more user friendly and therefore got more use.

Dearest DrJoek,
So I went to the dentist for the first time in a few years and the attractive young lady cleaning my teeth used what look like your basic pick/scraper but used high pressure water.
I've never seen this thing before...what the hell was that thing and how does it work?

Sounds to me that the Hygeinist was using a Cavitron
http://www.dentsplymea.com/products/images/cavitron_jet_sps.JPG
Which is an ultrasonic scaler. The Cavitron basically works by applying a high-frequency (either 25,000 or 30,000 Hz) oscillating magnetic field across a conductive metallic stack, in which is induced a reactive magnetic field. The mechanical vibrations so produced are conveyed through a rigid metal insert to a dental instrument tip, which is placed against the tooth deposits to be removed. The instrument delivers a steady stream of water to the working tip, which serves the dual purpose of flushing away the dislodged debris and cooling the working tip, which would otherwise become uncomfortably hot.
They are used for heavy cleaning and debriding. They work great

Dr Joe, or any dentists: my wife has a dental problem. She has a baby tooth that never fell out. The adult tooth is fused into her skull and never decended from when she was a child. The tooth looked fine and never really caused any problems, and was only discovered within the past 4 years. She grew up in another country and was poor and never went to the dentist until she was in her mid 20s. The tooth started getting a little loose and I just looked at it and see a pretty big cavity in the side up by her gum line.
I assume this tooth has to go. It's the insisor. Will the dentist need to grind down the teeth on either side of the tooth to make a bridge? Is that preferable? Let's say I get her there this week, what will likey happen in the visit? Will he pull the tooth right away and what about the space? When I cracked my molar, he ground it down and took an impression and fitted me with a temporary until the permanant crown came in. I was able to go home as if it was normal. Do they do something similar in her case? Thanks!

The most likely thing for a primary tooth without a missing replacement that has a cavity would be to have it extracted and replace it with an implant or to make a bridge using the teeth on either side of it. Yes they would have to grind the teeth on either side down to make a bridge. There are alternatives that dont require extensive tooth reduction for teeth that are in the front. A " Maryland Bridge" could be discussed. An implant would not require grinding the adjacent teeth and is the most aesthetic option but may be more expensive. but is the best option. The tooth that is extracted can be replaced with a fake tooth ( a Flipper) temporarily so she is not walking around with a gap in the front of her mouth until something permanent can be made.

Thanks Doc. Its going to go through insurance (Metlife), will they pay for an implant, or do they want it bridged? I am trying to prepare her, so will she walk out with something instead of a gap, right? I think I described it wrong. Its the lateral incisor.

It depends on each insurance companies contract with your employer. So Metlife has many different plans. Generally speaking they like to do the least invasive which would likely be the implant. the preffered method if you ask me.

Does the presence of the undesended adult tooth seem like it would be an issue with an implant?
