you're probably experiencing spasm of the pelvic floor. These muscles will spasm from time to time causing severe taint pain. It can be in reaction to urination or ejaculation or other problems. It's a sharp stabbing pain that goes away by itself eventually, only to happen again at some point.

You can get botox injections into the muscles which will stop the spasm but will need to be repeated every 4 months or so. Also pelvic floor physical therapy will help. A urologist is probably the best person to deal with this. Tell them you have "intermittent severe spasm of the perineum after urination" and they'll know just what to do. :-) Or just print this out and take it with you.

hope this helps!

Dr. Steve,

First of all, I love what you do and I know the listeners appreciate it. I have a (hopefully) quick question, and maybe you can give me some insight or advice.

Sometimes after I pee really hard or I've had to hold it a while - and sometimes just after a regular piss - my taint region seems to tighten up, and hurts enough to bring me to my knees. The pain doesn't linger for more than a minute, usually less. It doesn't happen all the time (maybe every other month or so), but it's been a fairly regular thing over the past five years (or so). I'm 26-years-old, in good shape, and have no conditions that I'm aware of (no STDs). I'm without health insurance like a lot of people, and would like to know if this is a common thing or if I should have it checked out.

If you so choose, I don't mind this question going up on the forum - but if possible keep it anonymous as I only post here sporadically, and don't want it to seem like I want attention. It's not a big deal if you'd rather include a name, though - especially if the problem is my penis is too long and it just gets tired from expelling the urine.

Many thanks! Looking forward to the next Weird Medicine!
