Dr. Steve,

I know you are not an urologist but I have this bugging question.

The research I have done states that vasectomy reversals have about a 40% chance (meta analysis) of being successful in producing off spring. I also know that is can be a rather expensive procedure not covered by insurance and costing up to $10k and there are few complications minus a small chance of increased rates of testicular cancer. Since I retired the Medical Center Library figured it out when I was up for a renewal 2 months ago so I can not access textbooks or get full articles through pubmed.

What does your information show as far as reversal success, cost, complications, etc? I am not too worried about having mine reversed since I can not ejaculate thanks to my psych meds.

My information isn't much different from yours...I'm just reading abstracts on pubmed myself. I've included an abstract from an international review below.

I think two things need to be said.

1) you can get your chick pregnant without a vasectomy reversal through sperm acquisition and in-vitro fertilization. The success rate of vasectomy reversal thus becomes irrelevant.


2) There's no need for you to tolerate retrograde ejaculation. What meds are you on? feel free to email me if you don't want to disclose a bunch of stuff here.

Vasectomy reversal: a seven-year experience.

INTRODUCTION: Since the demand for vasectomy reversal is increasing and many
populations in Brazil and other countries show distinct characteristics, this
study was carried out as an effort to determine factors and characteristics
associated with the success rate of reversal in a population in Southern Brazil.
PATIENTS AND METHODS: We reviewed 29 cases of vasectomy reversal performed over a
7-year period using the single-layer technique under microscopic magnification.
RESULTS: Mean patency, pregnancy, and birth rates were 75, 41.7 and 29%,
respectively. The patency and pregnancy rates were 92.3 and 38.5%, respectively,
for time intervals since vasectomy <10 years and 63.6 and 45.4%, respectively,
for intervals >10 years. Patency and pregnancy rates were 87.5 and 50%,
respectively, for patients who had their vasectomy performed by a urologist, and
50 and 25%, respectively, for those who had their vasectomy performed by a
generalist surgeon (p < 0.05). CONCLUSIONS: High patency and pregnancy rates are
associated with time intervals since vasectomy of <10 years and vasectomies
performed by urologists. There was no significant difference in the anastomosis
time between the first 12 procedures and the next 12 procedures. Copyright 2009

Recently switched from cymbalta (thanks norepi for hypertension) back to prozac 60mg/day
Buproprion 300mg/d
Klonopin 0.5mg 3x/d
Omeprazole 40mg/d
Naproxin 750mg/d
Tramadol 50mg/d

To sleep:
Trazadone 200-400mg qhs
Prazosin 1mg qhs

OTC daily:
A B-vitamin complex
Vitamin C
Fish oil capsules

PRN:
Sucralfate 1gm with heartburn
An extra naproxin 375mg

Quite a bit for a 35 y/o disabled veteran who is 5'10 about 165 and eats healthy. I have dealt with the delayed/no ejaculation thing since my mid 20s (at least 10 years) and have become use to it. I'd say 1 of every 20 times I get to shoot a little bit but that's it.

Prozac or Cymbalta can cause retrograde ejaculation. Tramadol and Trazodone have some funky adverse effects as well. In addition, the three of them can have some drug interactions; at the very least, talk to your pharmacist and have them run a drug interaction program on your med list. Your primary may be able to make a few adjustments.

Prozac is used by some porn stars to prolong time-to-ejaculation.

I have a good team and see the internest every 6 months and my psychiatrist every month. I know the pharmacists there really well. In all there is a lot of communication going back and forth, plus we have the best electronic medical records of any other institution I have worked at.
